Terms and conditions for Physiotherapy Practice
This agreement (“Agreement”) is entered into by and between [Physiogic Physiotherapy Clinic] (“Clinic”) and [Name of Physiotherapist] (“Physiotherapist”).
WHEREAS, the Clinic has the capacity to offer revenue-sharing physiotherapy practice opportunities to qualified physiotherapists, and the Physiotherapist desires to practice at the Clinic on a revenue-sharing basis;
NOW THEREFORE, in consideration of the mutual covenants and promises set forth below, the parties agree as follows:
- Practice Arrangement. The Clinic shall provide the Physiotherapist with a clinical space to practice physiotherapy services on a revenue-sharing basis. The Physiotherapist agrees to provide physiotherapy services to patients of the Clinic in accordance with the terms of this Agreement.
- Revenue Sharing. The Physiotherapist shall pay to the Clinic a percentage of the revenue generated by the physiotherapy services provided at the Clinic, as agreed to by the parties in writing.
- Investment Deposit. The Physiotherapist shall provide an investment deposit in one of the following three options:
- Low Budget: PKR [50,000/-]
- Medium Budget: PKR [100,000/-]
- High Budget: PKR [200,000/-]
The option chosen by the Physiotherapist shall be agreed to by the parties in writing prior to commencing the practice arrangement.
- Compliance with Laws and Regulations. The Physiotherapist shall comply with all applicable laws, regulations, and professional standards governing the practice of physiotherapy.
- Patient Care. The Physiotherapist shall provide high-quality patient care, in accordance with the Clinic’s policies and procedures.
- Access to Facilities and Resources. The Physiotherapist shall have access to the Clinic’s facilities, equipment, supplies, and administrative support necessary for the practice of physiotherapy services.
- Value Addition. The Clinic may offer value addition to the Physiotherapist, such as access to the Clinic’s network, marketing support, and training opportunities.
- Term and Termination. This Agreement shall remain in effect for 6 months, unless terminated earlier by either party upon 1 month through written/via email (info@physiogic.com) notice. The Clinic may terminate this Agreement immediately for cause, including but not limited to, the Physiotherapist’s failure to comply with the terms of this Agreement.
- Relationship of Parties. The parties agree that the Physiotherapist is an independent contractor and not an employee of the Clinic. Nothing in this Agreement shall be construed as creating an employment relationship between the parties.
- Entire Agreement. This Agreement constitutes the entire agreement between the parties and supersedes all prior and contemporaneous agreements, understandings, negotiations, and discussions, whether oral or written.
- Revenue sharing basis: The terms are as following:
70% Consultants share (Near Shaukat Khanum Branch)
60% Consultant share (from Physiogic Share) (ECC branch)
50% consultant share (if physiogic will give you patient)
- General points: You will be required to always maintain the cleanliness and hygiene of your clinical space. You are also responsible for providing your own equipment and supplies. You will be required to comply with all the policies and procedures of our clinic, including but not limited to the dress code, patient confidentiality, and ethical conduct. Your clinical practice hours will be determined in consultation with the management. You will be required to attend all meetings and training sessions organized by the clinic. You will not be allowed to solicit patients outside of our clinic premises. In case of any dispute, it will be resolved through mutual discussion. Please indicate your acceptance of these terms by replying this agreement in yes or no / acknowledged or not acknowledged.
